Power of Attorney & Healthcare Documents
Notary Ally provides mobile notarization for important life documents, including financial powers of attorney, advance healthcare directives, and related forms.
These documents are deeply personal, and we treat every signing with the care and respect it deserves.
Appointments are in person, and a current, non-expired photo ID is required. If ID isn’t available, two credible witnesses who know the signer personally may be used, as long as they are not named in or benefiting from the document. Once they have identified you, they do not need to be present throughout the rest of the appointment.
If the signer is currently in a Skilled Nursing Facility and signing an Advance Health Care Directive, a state Ombudsman must sign off on the Directive before notarization can take place. Please contact the facility to arrange for the Ombudsman in advance.
